ADVANCE DIRECTIVES

Your Living Will and/or Durable Power of Attorney for healthcare are Advance directives. These documents allow you to give instructions about your future medical care. If your advance directives are complete, please make a copy available for your medical record. Further information about advance directives can be obtained from the admitting/registration office and nursing staff. The nursing staff or case manager will be happy to answer your questions or assist you in filling out these forms.
